New Customer Application

New customers must fill out the New Customer Application, bring in proof of ownership (warranty deed) or lease agreement (if renting), a picture I.D., and pay a deposit for new service.  The customer applying for service must also be on the warranty deed or lease agreement and come in person to apply. 

*Application must be signed at the office or notorized

*Deposit payments accepted are Cash, Check, or Money Order

*Deposit for new service  *$150.00 for Owners 

                                                           *200.00 for Renters

                                                            plus a $10.00 membership fee ($160.00/$210.00)
